En fait  il est possible que ce soit un pb de formatage du mp4 qui ne plaise pas au lecteur... 
Mais bien souvent ce n'est pas cela

Le plus souvent c'est parceque soit le fichier, soit le répertoire n'a pas les bons propriétaires/permission. 

Ce qui marche (et rien d'autre !):
